Sore Throat: Age 5 and Over

Most sore throats are viral and do not need antibiotics. Our pharmacist uses the FeverPAIN scoring tool to check whether a bacterial infection is likely. A score of 0 to 1 means self-care is the right approach. A score of 4 to 5 means antibiotics are appropriate. If antibiotics are needed, we supply phenoxymethylpenicillin. Clarithromycin is used if you have a confirmed penicillin allergy.

Earache: Age 1 to 17

Earache in children is often a middle ear infection. Our pharmacist examines the ear with an otoscope before any treatment decision. Mild cases are managed with pain relief and self-care advice. Moderate or severe cases may need an antibiotic. Amoxicillin, co-amoxiclav, or clarithromycin may be supplied depending on allergy history. Very young children under 2 receive a more thorough assessment. Any clinical uncertainty means a referral rather than treatment.

Sinusitis: Age 12 and Over

Most sinusitis is viral and clears within two to three weeks. Antibiotics are only considered if symptoms have lasted more than 10 days without improvement, or if symptoms got better then worsened again. For earlier or milder cases, a nasal steroid spray can help. Antibiotics supplied, where appropriate, are phenoxymethylpenicillin or doxycycline. Severe symptoms such as facial swelling, stiff neck, or visual changes need urgent care.

Urinary Tract Infection (UTI): Women Aged 16 to 64

You do not need a GP appointment or a urine test. Our pharmacist takes a brief clinical history and supplies an antibiotic where appropriate. The antibiotic used is nitrofurantoin 100mg modified-release, twice daily for three days. Women with diabetes are excluded from this pathway following the October 2025 update. Pregnant women, those with recurrent UTIs, and anyone with symptoms suggesting a kidney infection are also excluded.

Infected Insect Bites: Age 1 and Over

Signs of an infected bite include increasing redness, warmth, swelling, and pus. Our pharmacist examines the affected area before any treatment. Flucloxacillin is the usual antibiotic. Clarithromycin is used if you have a penicillin allergy. Erythromycin is used during pregnancy. Signs of anaphylaxis, Lyme disease, rapidly spreading infection, or systemic illness are not within this pathway and need urgent medical attention.

Impetigo: Age 1 and Over

Impetigo is a contagious bacterial skin infection. It presents as golden-crusted sores, usually on the face, hands, or neck. Localised cases are treated with hydrogen peroxide 1% cream, applied three times daily for five to seven days. More extensive infections may need fusidic acid cream or an oral antibiotic. Children should stay away from school until sores have crusted over or until they have had antibiotics for at least 48 hours.

Shingles: Age 18 and Over

Shingles causes a painful blistering rash, usually on one side of the body. Antiviral treatment works best within 72 hours of the rash appearing. Come in as soon as you can. We supply aciclovir or valaciclovir where appropriate. From October 2025, shingles affecting the head or neck is a red flag for onward referral rather than pharmacy treatment. This includes shingles near the eye, ear, or nose. Truncal shingles remains within scope.

No. Pharmacy First is open to anyone who meets the clinical criteria. Walk in as a new patient and our pharmacist will take it from there. Where relevant, a consultation summary will be shared with your registered GP practice.

For conditions like UTI, where assessment is primarily based on symptoms described by the patient, some remote formats may be possible. Conditions requiring physical examination, such as earache, impetigo, and infected insect bites must be seen in person. Call us to discuss what works best for your situation.

We will not push you into a pathway that does not fit. If you do not meet the criteria, our pharmacist will recommend over-the-counter treatment, give you a safety net plan, or refer you to your GP or urgent care, depending on what is right for you.

Yes, please mention it. Previous antibiotic use helps our pharmacist check whether the same treatment is likely to work, flag any resistance concerns, and make sure you get the right course this time. This matters most for UTI and impetigo.

Not all of them. Age eligibility varies by condition. Earache and impetigo from age 1, sore throat from age 5, sinusitis from age 12, UTI for women aged 16 to 64, and shingles for adults 18 and over. Our pharmacist will advise if your child falls below the age threshold for a condition.

Our pharmacist records the outcome and shares a summary with your GP practice where relevant. If an antibiotic is supplied, you get the medicine with written dosing instructions and safety-netting advice, including when to expect improvement and when to seek further help.
